  • Mur's - done at the counter!??!

I was locumming at BTC (a branch in Birmingham) & I couln't help but notice the BTC pharmacist on duty "doing" an MUR at the counter! A number of concerns - at no stage did the pharmacist ask for permission to do this MUR & get authorisation from patient to pass any relevent info on to gp -
the MUR wasn't done in the consulation room (which was freely available at the time) - & should I have said anything...!!??
Any one out there have an opinion?
One positive thing was to be in a position to overhear another pharmacist providing an MUR & I think we definitely need to audit this service professionally & get feedback from clients.

On the same day a lady came in with a letter she'd received from BTC inviting her in for an MUR - which, after checking she met the appropriate requirements, I provided in the consultation room...personally & professionally I think the patient may have felt a little bit miffed if I'd had a "chat" at the counter? Surely the requirement for the MUR to be done in the consultation room was put in place for a reason?

    Absolutely right.

It is a disgrace that which you have witnessed.

And I know it isn't just Boots pharmacists who have done this

Did you witness the actual MUR or the arranging of an MUR.

I am a pre-reg trainee and my tutor regularly does the MUR's at the counter, he sometimes even gets 3 MUR's going at ONE time !... as he has 2 pree-reg trainee's (including me) and he get us doing one each as welll sometimes !
He NEVER asks the patient if they have the time or if they want to !!
He has to keep a VERY close eye on his MUR numbers as he does not have any trouble hitting his 400 a year target.. his only concern is that he regularly goes OVER his target and has been investigated for this before !!

I sometimes think the likes of Boots and Tesco would kill to have this guy on their books !!

This is quite an incredible cheat! 3 MURs at the counter, 2 are done by non-pharmacists pre-regs...

Well, that's what we become when we are target oriented.
              Are MUR our QoF points?

I must admit i have done MURs at the counter.

But here me out- you must ALWAYS carry out the correct procedure. I still explain what a MUR is, obtain patients permission and ask if i can pass the information to their GPs. I only do these for patients on few medications which they have been taking for years as the MUR process is pretty simple.

Of course you must always make sure this is done during a quite spell so there is'nt anybody listen in and make sure the consultation room is available so if what seemed like a simple MUR turns complicated the discussion can be moved to a more private place.

Im in no way trying to justify myself. Ive also heard of alot of pre-reg's being pressurised into doing MURs. If it is a simple one, in a consulation room with the tutor and with the patients permission no problem. But 3 at once??

She is paid a salary. She shouldn't be doing them because she isn't accredited for doing them. On what possible grounds can she charge her tutor for doing them? They are not contractually valid. This is fraud.

If it isn't reported how will anything be done?

What would the likely cost of reporting be to the pre-reg pharmacists involved?

The RPSGB should be monitoring this site and following up on the allegations.

                              Does only the pre-reg know?
