    Instead hospital and NHS number plus DOB. Some items already given out by another pharmacist on this script. I thought it was a legal requirement?

    Legal requirement clearly states pts name and address should be on script. Payment likely to be rejected.

      Very late to the party but it depends on what type of prescription this is. If it is a hospital outpatient prescription, the patient's name, and hospital number are sufficient patient details wise for dispensing. However, it will only be valid for dispensing at that Trust's hospital inpatient or outpatient pharmacy, not at a community pharmacy or at another Trust's hospital pharmacy. If it's an FP10, then it is legally not valid if the patient's address isn't on the script.
