    Error in dosette boxes

    Just wondering what you guys would have done in my situation today. So I'm working on a Saturday and the only person in the dispensary when a carer from a care-home comes to speak to the pharmacist. She then tells me that one of the patients that a dosette box was prepared for had donepezil in the night time box when it should have been in the morning box (the label also says should be in the morning). The carers would not alter the box due to liabilities and it is my first day in the store, no one around for advice and I dont want to go making a new dosette as not trained and time pressures etc.

    What would you guys have done?

    how can you not be trained to do dosettes???

    donepezil is usually at night time anyway, if you were pushed for time you could have told the carers to give it at night time for the period of the dosette box and it will be corrected for next time..once daily dosing, won't harm the patient either way!

    Me neither.
    Probably easier for us 30+ years in the job as we see these things trickle in and have the opportunity to develop a good technique.

    Besides, who will train the trainers?

    The key to sorting this out, and almost every other script issue, is to get hold of the original script. Failing that, phone the surgery and beg the receptionist to tell you the dose that was on the script. Failing that (and this WAS a Saturday, remember) search the PMR for what was dispensed and labelled on previous occasions.

    If a mistake was made, then it needs to be put right NOW. That may mean re-doing the tray then and there. Other time pressures, scripts etc are less important than correcting an error particularly whan, as in this case, the prescriber may have wanted the medication taken in the morning, i.e. its late already.

    When doing up a tray, the best way is to press out the correct quantities of drugs from blister packs into plastic measures and get that checked against the MAR/labels and the script. After that, insert into the correct places, and then get that checked. Obviously, if you are tablets short or over in your plastic pots then something strange is occurring.

    Seal immediately after checking, sign wherever, and check the bag label is correct.

    If you are a pharmacist you might want to take up strangenesses in the dosage with the GPs, but TBH unless its critical I would wait until the following week.

    i'd try everything in my power to avoid making up a new dosette box....seems un-necessary..its not imperative when the dose is taken (ideally it should be night time)..its not as though there was an extra unidentified tablet in there...
