How do I obtain an EPS Smart Card

Hi

I'm about to begin retail locuming. Can anyone tell me in detail how I can obtain an EPS Smart Card.

I've searched and searched the connectingforhealth.nhs.uk site, where I was told that there was info about how to obtain one, but alas I am non the wiser

I will need access to Newcastle, Northumbria, Durham, Cleveland, Sunderland....in fact most of the North East. Do I need to contact all of these PCTs? or is there a simpler way?

Any help would be appreciated.

One card from one PCT they'll need a picture and some ID - phone the most convenient PCT and ask.

Dont forget your password - and make sure you use your card regularly.

I just realised today that smart card actually expire after 2 years! (they never told me before...)
