Also say to the patient "I can see you are in difficulty with your medication timings, and its giving me serious concern I'll have a word with your doctor about it on your behalf".
If they get abusive you can turn them out after drawing their attention to your 'no tolerance of abuse' notice. Though you may get a lump of concrete through your shop window. And of course it will all be your fault.
