NVQ3. I need Prepayment certificates/reclaim forms.I live in Wales,scripts are free!

Hi, I'm doing NVQ level 3. I've had Unit 1 sent back, they want me to send a copy of a reclaim form and prepayment certificate. I live in Wales and we have no prescription charges so therefore don't have any of these forms instore. How can I get hold of them?? Arrghhh!!!!!

Suggest you resubmit without change, and request they update their database to fall in line with recent (and not so recent) changes to their own remit.

Ask your pharmacist to back your resubmission with a covering letter.

Thanks for your reply. I've thought of all ways around it but the bottom line is that I can't get hold of one, I've asked all our other branches but they threw out all forms such as this when prescription charges were dropped. I'm going to phone the NPA on Monday and tell them this and hope they accept my work.

The NPA cannot expect you to submit something that no longer exists. They are not the best at keeping up with legislative issues outwith the southern peninsula (generally recognised from the Celtic point of view as England south of the Watford gap). To be fair though, I have no doubt they will understand your position and respond accordingly. Please let us know the outcome.

I guess if the NPA insist on being difficult, you could ring a Pharmacy in England and ask them to post you one?

Quick update - The NPA accepted my portfolio after I included a short note saying I couldn't get hold of a reclaim form. Horray!
