Contract Application

Hi people,
I have been thinking about filling a contract application recently, and am a bit stuck as to what kind of information I should be including, what kind of data to include and also the structure of the supporting information. Has anyone done this recently or know where I can view a previous one on the net anywhere?

If I do do it, I want to make sure its good having seen the cost of submitting an application! (£750!!)

Just a little additional question for the experts out there, but does anyone know anything about the rurality test? in terms of defining current pharmaceutical service provision?

£750 is nothing to a GP practice who want to open their own 100 hour pharmacy.
Where they are going to find pharmacists from to cover 100 hours I don't know.
There aren't any I know who would work there.

Perhaps they know something we don't about remote supervision and use of ACT.
Maybe the reception staff will also help out...
