    BTC wanted Zocor replaced by the generic on the day that the generic became available. Estimated that if not done then would lose £150,000 of profit. Forgotten whether daily/weekly/monthly. I understand the container allowance also supposed to cover MDS as well.

    Yes, it's supposed to cover the cost of all containers. Whether it actually does or not depends on whether you buy cheap or expensive containers. Some wholesalers offer better prices than others.

    I did hear that the container allowance would go if we moved to original pack dispensing (OPD) and only paid when non-OPD quantities. A new allowance would be introduced to cover syringe/spoon on oral liquids.
    Going back a few years - must be very late 90s.
    OPD never happened...

    Can someone explain how we get paid for items please.

    Part V111 has generics and prices so i am assuming these prices are what we get reimbursed NO MATTER WHAT WHOLESALER is used.

    How are brands paid for?

    Manufacturers list price as per C&D is usually used which is based on the PPRS price (I think).
    PI are priced pro-rata on UK price.
    There is a PI pack rule - its in the new PSNC Dispensing guide for 2012.

    However, and sorry if this is boring...
    If you dispense a brand against a part VIII generic you will be paid the generic price.
    Some part VIII Cat C are based on brand until the generic becomes available in a certain number of wholesalers.
    Then the price can drop.

    If you are working in a chain your system may have brand equalisation deals.
    If you are not in a chain and your system defaults to a brand when the generic is off patent CHECK BEFORE YOU DISPENSE THE BRAND. Otherwise you could be losing the difference in price - this can be loadsamoney.
    Do this if you are in a chain as well in case someone has set the default.

    If I dispensed co-amoxiclav instead of augmentin if the script asked for augmentin, is that illegal then, would ppd send script back. I seen it happens in practice but I was under the impression you always supply what doctor asks for

    Look at some listings in part VIII you will see some of them have a brand listed in the far right column.
    That's what the price is based on.
    Most of the time it is because the brand is on patent.
    When the patent expires there is usually a short time when you can dispense a generic and be paid brand.
    However, like all things in pharmacy the payment system catches up and you pay this back in the end through category M.

    You can dispense generic if brand is supplied in an emergency;
    I've done this on saturday but have explained the situation with the patient and asked for a replacement script.
    The patient is quite free to decide to take their prescription to another pharmacy.

    I would encourage all our medic colleagues who are on home visits to prescribe generically wherever possible.
