National Patient Safety Agency advice
Just wondering if any of you ask
1)To see patient WARFARIN YELLOW BOOK? e.g doses, when is their next blood test?
2)To see patient METHOTREXATE book? weekly dose?

Ask when last letter was and next test due

Not the book just ask to confirm that recent tests done and tests due and that dose is known.
