Hi, I have 2 questions regarding a requistion for a POM. Im not sure if my answers (in blue) are correct or if I am still missing something? Any help or advice would be greatley appreciated
  1. List the information required on a requisition for a POM if the pharmacist is to retain this order rather than make an entry of the transaction in the prescription-only register?
The requisition has to be ordered in writing and include:
*Date
*Name, form, strength and quantity of drug
*Name and address of purchaser
*Purchaser’s profession, trade or business
*Purpose for which the drug is requested
*Signature
  1. If the requisition was for Pethadine tablets what other information would be required and where would a record need to be made?
The form that should be used is an FP10CDF in England. It is lawful to supply from a non standard form as long as all of the legal requirements are there.

Other information required would be for the total quantity to be written in both words and figures.
A record would also need to be made in the CD Register.
