Buttercups Module 2 assigment 1- types of prescription forms used?

Hi, im currently in the process of registering in the UK as a pharmacy technicia. I just need to complete a few modules via buttercups, as i trained as a technician overseas. My time in the UK i have only worked in hospital pharamcy so I have had nothing to do with the Drug Tariff or prescription types used.
I have a question which i would appreciate help with, or a reference to point me in the right direction to find the information i am after.

Fill in the following with regard to types of prescription forms used by different prescribers:

Doctor-
Restrictions of their prescribing (state both private and NHS restrictions where applicable)?
Can they prescribe on NHS forms?
Can they prescriber on private forms?


Dentist-
Restrictions of their prescribing (state both private and NHS restrictions where applicable)?
Can they prescribe on NHS forms?
Can they prescriber on private forms?

Community Nurse Practitioner Prescriber-
Restrictions of their prescribing (state both private and NHS restrictions where applicable)?
Can they prescribe on NHS forms?

Pharmacist Indepentent practitioner-
Restrictions of their prescribing (state both private and NHS restrictions where applicable)?
Can they prescribe on NHS forms?
