2) If not in Dt, then is a special and you can claim for actual cost, normal dispensing NHS fee, and Out of pocket expenses, ie the post and packing.
3) I have never seen this before. Normal saline is 0.9% and 5% would be hypertonic. Script most likely from hospital. Proof of costs will need to be attached to script, ie supply invoice.
Perhaps someone else has other comments.

sounds about right....just calim back the costs that the hospital/specials people will charge, plus your own costs.....i'm assuming to make it up they will just dilute the 30% NaCl injection and decant it into eye drop bottles.
We order this special from Martindales regularly for several patients at a cost of £57.00 per bottle and £14.70 delivery charge!!! There's also an ointment but can't remember the price of that. And welcome to the forum....
As this isn't listed in DT, you would need to endorse the supplier, trade price and claim your OOPE (£4.95 P&P) You will recieve your dispensing fee automatically.
