    wrong number of tablets in box...

    Sure you've come across, patient comes back (either with or without product), saying that they were short of x number of tablets. Easy mistake to make, if split pack given out or split pack was not marked to indicate it was split,etc. But what do you do in this circumstance, or what to do if it is say, DHC, co-codamol, temazepam. If you did trust the patient and gave whatever tablets were short, are you able to defend this if you get any comeback on it?

    If you can justify your decision (to give it or to not give it upon request), and you can write your reasoning in an intervention log and on their PMR (in case a future pattern develops), then I don't see how it can come back on you. Your only reason to make up a supply will be based on your duty of care and you would only be acting in the best interest of the patient. If you don't write it down it never happened, so log it and back it up to cover your back!

    As for temazepam....if you did not check it off, I would definately not rectify it at all!!! Unless you can physically see from your stock levels and what you've given on that day that you've checked off an error, I would stay away and request a new Rx!

    Leaving out CDs that are S3 and above...
    Make a point of checking anything with a Benzodiazepine, 'Z' drug, higher strength/potency opiate (non-CD) and others.
    Either while it is counted (if they are loose) or looking in the box.

    And for certain patients, have them check before they leave the shop.
    I'll give somebody the benefit of the doubt once but after that I get them to double check they have what it says on the tin.

    I ALWAYS double-check quantities of CDs, then write, say, '28', circle it, and initial the note, INSIDE the flap of the box. Never any arguments then, as I have extra proof that I checked it.
