    Methadone sugar instead of sugar free..

    Patient came to no harm, did not notice, it was picked up by another pharmacist..

    I have been reported to the bosses of Froots...what is the likely outcome now?

    I'm an agency locum..

    No harm done really.
    You have probably been reported as an '*rse' covering exercise. That's the nature of it these days.
    In my view something like that doesn't need to go to the top.
    We have had to substitute one for the other regularly when delivery has not arrived on time or not at all.
    Pharmacy I work at marks up SF scripts "S/F" on top margin with highlighter. Similar on bottle a label is stuck on the bottle "S/F" Highlighted.

    If they write to you, write back explaining that you have reflected on your practice and what measures you are adopting to prevent recurrence. But don't apologise in the letter. If there were cirumstances, such as inadequate staffing so you felt under pressure to 'over mult-task' get that in there.

    we colour ours in pink for sugar free, green for physeptone ( yes the local clinic thinks its cheaper )

    this particular branch has about 20 addicts and I had to make up the dose as and when they arrived..I had a healthcare assistant with me but no dispenser. I will be sure to take your advice.

    I know it's a bit late, but I've only just read this post. In my own pharmacy (those were the days...) I always made the methadone up the previous day, as I had a queue outside when I opened at 8:30 and making it up on the spot was asking for trouble. As a locum I find that most Boots branches make the meth up a week in advance. I have occasionally given S/F for plain by accident, but mostly when I'm working in a pharmacy that makes it up the same morning, which only goes to prove what I said at the beginning!
