    Methadone Collection Issue (Error?)

    Hi, Thought I'd lay out the issue (I'd call it an error in terms that the wrong person may have received the prescription) and see how the rest of you would respond to the situation.. Did seek advice at the time from a colleague and our operational support also who took the same view as myself.

    This resolves around two methadone patients (A + B for these purposes.) I am the regular pharmacist and know both. Have been there 4 months but this practice had existed before I was there and had no previous issues.

    Patient A is supervised and came into collect their medication. They also presented a signed and dated note from Patient B stating to give the prescription to patient A. This had last occured 3 days ago as well as quite often previous(Always keep at least one previous note for reference or incase something crops up). At a quick glance, the wording was standard and the signature was fine so no reason for suspicion. The checked prescription was issued to patient A, who was acting normally. Even responded when asked why patient B could not attend, that patient B was not feeling well. (this is the usual reason but who am I to judge?)

    Later in the day, Patient B came in to collect prescription. I explained patient A had collected it. Patient B explained that patient A must have forged the note. Compared signature to previous kept notes- very similar and patient B is a bad writer which doesn't help.

    Where would you have gone from here..? (Ideally you would not have got there of course, but play it from here)

    Besides panic!! I guess the most obvious route would be to try and get in touch with patient A and see if he still has the methadone. Assuming patients A and B are friends and know each other quite well?

    Try and ascertain if patient B has already had the dose and is trying to get extra out of you..again hard to do without bringing out accusations...

    tricky situation to be put in..get drug workers for A and B involved..other than that i can't be of much use, i'd be pretty rubbish in community!

    I did phone Patient A. Patient A said the opposite of patient B. Patient A suggested that patient B had received the methadone and sold it. No resolution reached- accusations flying from both sides. Both were agreed that some form of break-up had happened though..

    Difficult to see who is telling the truth..i guess the best thing to do to avoid it happening again for A and B is not to dish out the prescriptions to the other persn and they will need to collect it persoanlly or they don't get it! simple as!

    Could have called patient B's bluff and say "Oh, I'll just ring A and ask him/her to come in and explain". If B started acting or looking nervy then you'd know they were working with A to try and pull a fast one. If B was fine with you ringing up A then you'd know A forged the original note. I suspect the culprit or culprits were probably waiting for a suitable time over your first 4 months to gain your confidence after trying out this scheme. Stick to your guns and be forceful if necessary.

    Well, I did try to call bluffs too, I got patient A's number from B. And patient B was quite happy for me to phone the police if required. This happened 5pm Saturday actually. No doctors available etc.

    i'm guessing its probably not the first time they have done this, so to call their bluffs may be a little tricky, and they may be quite good actors...

    Either way you wouldn't be able to dispense any methadone without a script,

    i'd let them fight it out amongst themselves lol

    Glad you've added this statement; this law was the main basis of my decision. I could not ascertain the location of the current medication or prove that it had been stolen. In turn I could not prove it had not been taken by patient B.

    If this had happened before it wouldn't have been allowed to continue if such an issue came up.
    The only time I've ever seen one methadone pt collect for another was one woman collecting for her husband.

    i guess it won't be happening now....and A and B only have themselves to blame...and prevented others from doing the same..they could well be the genuine ones!?!?
