    To name and shame..???

    Considering we are living in a socety of a "no blame" culture, which in theory sounds nice, but in practice is probably quite hard to do. Its always easy to point the finger when someone has done something wrong,

    But is it right to name and shame people when they have made mistakes? In my place of work we have recently moved to an error reporting system where the name of the person making the error is recorded. previously it had been done anonymously!!

    Since we are on a no blame culture is it right to name and shame. I can see that it is easier to give feedback from a training point of view if the same person is making the same mistakes (guess i may have answered my own question)

    just wondering what everyone elses thoughts were???

    What I saw, the name and shame culture creates corrosion whereas a supportive culture mostly creates win-win situations.
    I like the win-win philosophy.

    The way the RP legislation works seems to me to be incompatible with a "no blame" culture. The way it is currently being applied by the police, for instance, is to find an easy target to blame.

    And as for the criminalisation of one-off dispensing errors -
    ....just my opinion

    Its a blame-culture we live in, due to the compensation seeking nature of modern society.
    The media also quick to look for someone to blame.

    we have what is called a "near miss risk log error" book, weve had it for years now and anyone who is anyone goes in the book if they make an error, which could be somthing like wrong quantity, name, strength, drug etc, at the end of the week the pharmacy manager writes a "next steps report" and discusses it with the people involved, none of the dispensers seem to mind but the manager gets a strop on when you put her in the book *giggles*

    When a product has been dispensed and initalled by a dispenser or pharmacist but found to be incorrect in the final check ie: wrong name/address, quantity, product, basically any error that is picked up before the product goes out to the customer

    I have read documentation with near miss reporting schemes which included things as trivial as picking up 250mg amoxicillin instead or 500mg, but realising before putting it down on the bench, and rectifying your own error instantly. I guess it could point to poor shelf layout etc - but if every trivial incident holds up dispensing by 5 mins while you fill in a questionnaire then chaos rules. I am afraid I ignore these schemes now.

    well thats ridiculous, thats not a near miss, its only a near miss if its been labeled and initialled, i would ignore it aswell
