Exactly, I remember when I moved from the shop floor to dispensary (many moons ago) and training was very limited. I am by no means condoning the 'being distracted while working' but from experience can understand the lack of knowledge and in some cases unawareness of the consequences.

Surely it would have been good practise to ditch the MAR sheet and use the Rx when checking the CA, even if only to check the CD.
