    who should complete a dispensing error report?

    who should complete a dispensing error report?
    1) dispensing technician
    2) clinical governance lead
    3) pharmacist who discovered or informed about the error
    4) branch pharmacist
    5) other?

    what if it is a busy pharmacy and you are there for a day,
    priority is finding the script or carry on checking calling back and delivery?
    do you trust the branch pharmacist to follow this up?

    What constitutes a near miss?
    I've been told that it is picking up the wrong box, even if you put it down right away.
    Sometimes you have to pick up the boxes to read the ingredients properly.
