    what do you do if you instead of me ?

    In this matter we will discus the prescription errors which met us in the pharmacy, and putting the suggestions and solutions to correct these errors
    let,s start with example :

    patient came to me with prescription of these drugs
    1-plavix 75 mg ( clopidogrel )
    2-omeprazole 20 mg

    what do you do if you instead of me ? will you give the midicine to the patient or not ? and why ?

    Depends, has patient has this combination before?

    If patient is on a PPI then aspirin for anti-platelet use is not suitable either so clopidogrel being used I assume.

    BNF doesn't list an interaction between clopidogrel & PPIs.
    MHRA has "Reduced effect of clopidogrel - concomitant use not recommended unless considered essential"
    Not a safety issue that I can see if patient is not on any other medicines.

    We would now ask the Dr to substitute the omeprazole for ranitidine, which is a safer option interaction wise.

    Ranitidine has lot of interactions though... so if the person had no other meds it might be ok but a lot of people on clopidogrel are on other drugs. Pantoprazole is a PPI which can be used
