either you haven't worked in a busy stressful environment or are making this comment tongue in cheek, as every pharmacist who says they have are self incriminating themselves (LMAO).
I , myself haven't made any errors with drug, strenght, dose and quantity the only ones I catch myself doing are e.g. leaving out a pack of paracetamol from a large order (but it finally does make it's way to the patient), and once I gave out a Novopen with the wrong size.
Main error is that correctly dispensed medication is given back to the wrong patient.
Most patients roll their eyes like you are stupid when you ask for their address after calling their name.
One well to do lady called in and asked "Is my prescription ready?".
When I asked of her name she said "Its on the prescription".

I have been lucky enough to do my pre-reg in two different branches. One barely did 200 items a day and it was a struggle to keep awake, the other did a max of 850 items a day and it was a struggle to keep up.
In my humble opinion boredom is just as bad for errors as being overworked.
I just hope my new branch is halfway between the two

3 years ago I didn't make an error.
I dispensed what was on the script after the usual routine checks.
I will get summonsed to a coroners court later this year and then have to face the RPSGB.
(Yes I'm in the PDA who have been fantastic but it still scares the cr@p out of me)
