Re: Should i give 250 ml or 500ml Gaviscon bottle

How do you forget to label something ? It seems as though you never had a proper SOP to follow through your scripts, i.e identify script, legal/clinical checks, prepare label, get stock out, check stock, check label matches script, STICK LABEL ON, final check, etc, etc.

Excellent point..just goes to show how easily we can make a daft mistake, and never quite understand why we did it! You have just proved how important the dispensing exams are.

Most importantly...I bet you'll never do it again.

    Welcome to the club.

It's easily done Nik..especially at chaotic times when SOPs would not be the first thing on your mind. A friend of mine once 'dispensed' his bottle of Tippex by mistake in a similar dispensing frenzy..luckily, however, he didn't label it!

The moral here is to 'learn from your mistake', and fair play to LIMA09 for admitting it.

Please don't forget this is the 'Newbie Questions!' section...

In a factory. an empty pot of what was being labelled was used for glue. Naturally it got labelled and sent out to return as a complaint months later.

Fair enough Fleeg I may have been a bit harsh in my post. Hopefully there was no offence taken.

Rule is read label against script, then against item. Ie prednisolone not propranolol.
