portfolio of evidence nvq3

hi,
im on my 11th module on my nvq level three and im finding the portfolio of evidence difficult as i cant seem to find any 'evidence' can anybody advise on how they have dealt with this? and also how to write it correctly as i have had my last two sent back for amending.

Yeah its unit 11, i just cant seem to do it.. I need to get evidence of pharmaceutical enquirys such as a surgery calling the pharmacy for information on a drug (which never happens) or when a patient asks for information on a drug.. and i dont know what i could class as an enquiry.. Im very confused to say the least lol xx

hi lve just completed it I just asked a few patients if they had used the med before and if not offered to explain then wrote up the info l gave ? hope thats ok

about a patient asking on a medicine , i think i done a ventolin an volumatic for a young child , the mother asked to show her how to use it, or it could be giving advise on bucastem tablets - how to take them , the surgery sometimes rings our pharmacy querying if a medicine is available, not sure if you have already done this but thought id post to incase it would help other reading x
