dose regimen of the same drug IV and oral

I have been having this question in mind for quite some time: IV medication enters the blood directly, but why are some of them being given a higher dose than their oral counterparts? Some are even given more frequent than the oral ones eg. oral cefuroxime and IV cefuroxime.

Reason depends on a number of things such as indication, age of the patient, concurrent hepatic/renal impairment etc.

Partly because IV administration is normally reserved for severe acute illness, which may require high doses.
