    how to ensure pharmacy record is 100% correct?

    in the morning, done an owing for fluoxetine 30 caps, later found out there is 10 capsules on shelf, a new label is done with an owing of 20, all seems fine. stock arrived later afternoon, when another staff went to make up the owing, no record was found on patient medical record.

    lucky to find this out on the same day and sorted it out but there could be a few others that happened and you may not notice (tip of the icebergh)?

    how could you ensure this will not happen again?

    are we all exempted from punishment for not keeping a 100% pharmacy record if anything like this happens?

    Did you do the owing on the pharmacy computer?
    If you did a manual owing there will be the carbon copy in the book.
    Not sure how long you have to keep owing books for, but we seem to have to keep anything we do for ever.

    Sound like user error to me.
    What PMR do you use?
    When you say "No record was found" do you mean that the owing was missing or that there was no record of the dispensing at all?
    When the new owing was done was it put through as a partial dispensing of the original owing?

    yes, it is an user error. Have to do a new owing to make sure patient medical record is correct.

    Another day an user decided to print a new label from the computer, she deleted the original entry and make a new entry for the same medicine but with a slightly different instructions.

    I know it is all down to staff training but sometime retraining just does not work...

    Pharmacy Manager allows you to edit a label and then reprint. Control Q then Control M.
