Unhappy Another Part 3 CPPE Assessment thread

    Hi there,
sorry for opening another thread, but I've failed part 3 twice now, and I'm really worried about having to start all over again...
I just wanted to ask a couple of questions:
    * Do you need to ask the patient's permission in order to send a copy of the MUR form to a District Nurse? (I've chosen True)
    * A Patient on digoxin looks drowsy, should you recommend the GP to have his digoxin blood level tested? (I wanted to answer True because it's a symptom of digoxin toxicity, but reading a document produced by a lpc had me answer False ... the reason not to recommend a GP what to do is "because you don't have full access to the PMR")
    * If a patient is on digoxin and senna, and the District Nurse prescribes him some Manevac granules, should I contact her in order to make sure she knows that the patient is already taking other medicines? (Last time I answered Yes, this time No ... I thought perhaps she is supposed to know which medicines the patient is taking...)

The lack of feedback with this assessment is really frustrating... I've heard that the medway one is more straight-forward, is it true?

    Thank you all,
    Bye

    Matt

    Hey Matt,

I was pondering on doing CPPE too but the lack of feedback and the lock down for 6 days are putting me off. I spoke to several of my friends who have completed the MUR via Meadway and they all say it is alot easier. You have to pass three case studies out of six and you have six attempts. They even point out the issues for you to address, you just have to watch a video and you have the patients PMR and Medication history at hand. 45 mins to complete the whole process so I think it looks a lot easier!

    Hope that helps

    Thanks for your reply asashaikh,
it looks like an extended version of Part3 CPPE MUR.
As I've already paid I'll try to pass CPPE, but in case of failure I will surely try the Medway.
By the way, what do you think about the questions I posted?

    Matt

Do you need to ask the patient's permission in order to send a copy of the MUR form to a District Nurse? (I've chosen True)

I would choose false because of the fact that district nurses can referre patients for MURS but i dont think that they would require a copy of the MUR form to make adjustments because of the fact that they cant really make changes to the patient medication unless they are supplmentary prescribers. From this scenrio i dont think that the district nurse is a supplementary prescriber.

    * A Patient on digoxin looks drowsy, should you recommend the GP to have his digoxin blood level tested?

In this cause i would recommend that the patient goes to see the GP as you mentioned that drowiness is a symptom of dixgoin toxicty so its important that the patients serum levels of dixgoin are mointered

    * If a patient is on digoxin and senna, and the District Nurse prescribes him some Manevac granules, should I contact her in order to make sure she knows that the patient is already taking other medicines?

I would look up in the BNF if there is an interaction between Manevac granuels with digoxin or senna. if there is then i would inform the district nurse that the patient is taking other medication.

    Hi Ghajini,
you've been really helpful, thank you for that.
In order to pass the CPPE MUR you have to:
    * Being able to use the BNF. Many of the answers for part 1,3 and 4 related to the patient's pathologies are in there (for the rest, keep a window open, Google is your friend also in this case! And keep some useful site bookmarked)
    * Study a couple of boring but indispensable pages in part VI-D of the Drug Tariff: this is the key to passing part 2 of the assessment (it's also available online at this link http://www.ppa.org.uk/edt/December_2011/mindex.htm )
    * Study the questions answered in this forum

*Being LUCKY: sometimes the questions were not really straight forward. I can just advise you on using good sense and to never forget that MUR is not a clinical review but a review on medicines. Also remember that MUR and Prescription Intervention Service are in fact the same service, just triggered by two different events (in case the question states that a patient can't have an MUR because he already had one the previous month, in fact Prescription Intervention MURs are not subject to any time limit).
    * Ask a pharmacist (that would in turn ask a patient) if you can assist a real MUR, it really helps you focus on the true nature of the service (Which medicines are you on? Do you know what they are used for? How do you take them? Have you got any side effects (eg mm spasms with statins ...) ...).

These are the things that comes to my mind at the moment, if you want to know something else feel free to ask me.

    Hi Ghajini,

    the most important ones are:
    * The Online Drug Tariff: http://www.ppa.org.uk/edt/December_2011/mindex.htm
    * the PSNC MUR resource: MUR · Pharmacy Contract & Services · PSNC
    * the Google Homepage: Google
    Also consider:
    * the online BNF: can't find the link as I'm in Italy at the moment so I can't access it
    * all the threads related to MURs that you can find on this forum

    Hope this helps, Good Luck

    Matt
