    How about keeping a dispensing error log, this should be done anyway, but make it more visible. Also recording initial of person making errors. Then highlight errors (generally, don't point her out) being made in the dispensary to the pharmacist, line manager, area office, area manager etc...

    Unfortunately, some people don't take the dispenser job too seriously, they find out early they can get away with making errors or doing naff all, so don't bother improving, just like this assistant who is hiding behind the training excuse.

    I'd second this approach. It's almost like a "Name and Shame" technique - for those that care it works, for those that don't it allows the line manager to have a log of evidence to support any action taken.

    If you are a registered ACT then you are also bound by the code of ethics. Being a professional in your own right means that you have responsibilities and duties. If you feel that something about the dispenser is not right then you need to inform the superintendent of the issue. Simply ignoring the situation and refusing to check would essentially be akin to burying your head in the sand.

    The dispenser in question needs to be informed that SOPs are there for a reason. Working outside the set SOPs (assuming that they have been signed and understood) means that they may also carry the blame when something goes wrong.

    Perhaps a quiet chat may be sufficient. If that does not work, then you, as a professional need to behave as one and make a decision as to whether it is safe and take the appropriate action to safeguard the public.

    In BTC in MK had monthly report on errors. Common one was dispensing split pack as whole one. Next was wrong strength of bendro.
