Having done this the nurse in question soon phoned back in a state of panic, "you haven't given it have you, it was meant to be for 150mg?" I think she just clicked on the first line that said Plavix without looking at the strength.
Watch out for this in EPSR2.
Just as easy for the pharmacist to 'click right through' as well. Have had a 'hands on' try of it with test data and it is very easy to become 'computer numb'.
Doing everything from a screen AND keeping 100% vigilant will take some getting used to.
Where am I?; In the Pharmacy.
Who are you?; The new Number 2.
Who is number 1?; You are number 6.
    What do you want?;..................

I guess it's another case of judge not least ye be judged. I stand guilty as charged.
There are failings everywhere and someone will always notice the failings of others but seldom their own. Bieng able to admit a mistake was made and then to rectify that mistake is not a crime. There is always a lesson to be learnt from someome elses mistakes. The one I have learnt from this is don't expect everyone to work to the same standards I set for myself.
The nurse learnt to look more at what she was prescribing, the staff learnt to be more vigilant of warnings. The dirty linen was all clean as far as I was concerned.
