As far as I can tell there is nowhere really rural like Australia more than a few hours drive from a big town or city, unless you count the islands. I believe salaries are slightly higher on the islands, however living is also more expensive and you have the hassle of travelling to the mainland to go to the movies.
In the NHS the islands living allowance is only a couple of thousand per year, less I think than the London living allowance.
In some other areas I have heard of individual pharmacists being offered really good salaries (third hand so I don't really know) but this probably depends on the individual and what they have to offer more than the location.
Rural areas are like any other place: people weigh up what the place has to offer, staff, training, remuneration and entertainment. If it is really unpopular but pays well it is a safe bet that it is not a good place to work. If you are looking for quite a lot of money and can live without the lifestyle the plan is probably to take a contract in the middle east, an added advantage is that since you can't booze your money away you leave with some nice savings.
