    Jobst Elvarex Custom Fit

I'm a newly qualified, I've received a fax prescription for Jobst Elvarex Custom Fit code L1-01-06 (lympheodema garment).

No size or measurement is annotated on the prescription form and the prescription is for a patient residing in a nursing home.

Now I'm unsure who's responsible for measuring the patient, should the community/district nurse come out to see the patient, take the measurements and pass them onto to the pharmacy or does the responsibility lie with the pharmacy.

Any help will be greatly appreciated.

Nbr 1. Are they allowed on NHS script? ie, in Drug Tariff?
If pt in nursing home, must have a nurse somewhere to take measurements. Heres the measurement form:

This product is allowed on FP10, but why have you received a fax? I personally dislike faxed prescriptions unless there is a very good reason, the actual prescription tends not to turn up.

Since the patient is in a nursing home, a nurse needs to fill in the measurements on an order form. This should be brought to the pharmacy along with the prescription. I would never get involved in measuring a patient for such as complex garment. This is normally done by a specialist lymphoedema nurse. You need to contact the home to let them know to bring the order form in.

L1-01-06 is the Class 1 thigh high stocking.

Here is the lower limb order form....
