    Interactions!

    Hi all
    Just a quick question. Can someone give me a couple of good examples of 'drug-patient' and 'drug-disease' interactions? I really appreciate your help with this!

    It is my pleasure to help you with your homework.

    Drug-patient - avoiding medicines in patients who do not have specific enzymes to metabolize the medicine e.g I remember reading somewhere that some people lack the CYP2D6 enzyme in the liver - therefore these people cannot take drugs that require those enzymes to metabolize the drug especially Codeine!


    drug-disease - e.g avoid some medicines in patients with liver problems or kidney problems - is it a good idea to give paracetamol to a patient with severe liver failure?

    Not necessarily a lack of the enzyme, more the fact that some people are "poor metabolisers" and thus unable to convert to morphine.

    Difficult to say avoid paracetamol in liver disease - would prefer over NSAID's (fluid retention - ascites, bleeding risk in the context of raised INR, reduced GFR) and opioids (encephalopathy risk from sedation and constipation). However could add weak opioid to paracetamol if needed.
