i make a dispensin error everyday my worse one was propanolol instead of prednilsone. she was happy as her anxiety had resolved and no longer needed to smoke an ounce of weed every week

This was same error made by Mrs Lee. She has a criminal conviction. Count yourself lucky. What was the dose? If had been 8 tablets then you too could be before a court,

I just want to say dispensing error occurs because of pharmacist multi tasking which can occasionally lead to skipping basic checking step and hence missing a mistake, from my own experience the worst problem I have found is, when a pharmacist is forced to dispense and checking because it is so easy to overlook a mistake!
