Anyone ever seen the combination, thought it would be one or the other for balance of diuresis. Also if loop was casuing low k all the time, you would switch to spironolactone no.
Combination is sometimes seen together, particularly in heart failure and for treatment of ascites due to alcoholic liver disease. Spironolactone on its own is not a potent diuretic to cover for the indications furosemide has. If pt's were consistently getting low K+ would consider switching to something like co-amilofruse.
