    If I understan it correctly, it says "this may be a specialist option in patients with heart failure who are still symptomatic despite optimised ACE inhibitor and beta-blocker therapy" hence, it is basically an option a specialist might explore to see if it works. You should ask one of the doctors in Hospital during your rounds ???? In community the GPs blindly follow what the consultant prescribed. No questions asked there. They seem to be too busy to question the consultant and then take responsibility for anything that may go wrong

    In the short time I've spent in cardiology during pre-reg, I've not seen these drug groups prescribed together and I'm sure more senior pharmacists will no doubt question the consultants decision.
