    azacitidine+dialysis

    Hi,

    Does anyone have any experience of giving a patient who is on dialysis (don't know how often) sub cut azacitidine?
    As far as I can see, there is no data for patients with renal impairment, other than that stated in the SPC, or for dialysis.

    I think it is cautioned in renal impairment as it is primarily renally excreted (plus active metabolites), it also seems quite nasty on the kidneys raising SrCr and potentially causing ATN. However, never seen it used in a patient on dialysis. Its also quite new and all the trials excluded the renal patients. What do the company say? Not seen anything about it on the Renal Pharmacists Group forum either.

    If I were to have a gamble, given its size, I would have thought it is almost completely dialysed out meaning that the dosing would be post dialysis only - but that is a best gestimate and most certainly shouldn't be used as a dosing reference.
