    In practice its not eally used as the sedative of choice...i've seen it being used a few times for severe acute asthma.

    Also in the management of asthma exacerbation,SABA is used either by nebulizer OR MDI ( in the same guideline)

    Ah-ha.
    Second line of my post implied that we are watching things and making sure technique is good.
    Less risk of acute phase developing
    Use PMR as indicator of bad technique.
    Lots of blue, few brown.

    yeah i get your point.....and its true...but what about someone with brittle asthma?

    and it could be that its not bad technique, it could be sub-optimal treatment on the brown inhlaer front....

    Point taken - thing to do is to talk to people and put notes on the PMR.
    Counsel up front - a lot of people still frightened of 'steroids'.
    No - we won't catch everyone - but one less per pharmacy makes it a win for the NHS.
