    Question nitrofurantoin 100mg

    Hi,
    Just wondering whether pharmacist can make this decision:
    The patient has the prescription for nitrofurantoin 100mg capsules M/R (Macrobid) and he was sent from another pharmacy but they did not mention M/R over the phone... the patient is very upset cause he had to drive all the way... on the shelf we have those capsules but NOT M/R ..... can I, a pharmacist (I'm not one yet), dispense nitrofurantoin 100mg capsules (Macrodantin) changing the dose from BD to QDS, assuming that no doctor is available and patient is very angry and in pain? if not why not, if yes then what shall I do to make legal because ethicaly I'm putting patient's safety and wellbeing first already... I think... pls what do you think....

    i wouldn't change it without talking to the doctor. plus not knowing if there is a conversion between MR and normal release nitrofurantoin. i would have thought you would want to change it to 50mg qds rather than 100mg qds. so patient still gets the 200mg daily?!?!?

    if you were to dispense the 100mg qds you would be going against the drs intentions.....i would think that wouldn't be allowed, unless you took a verbal order off a doctor or got a new script.

    As for the patient being angry, its not your fault! its the way the cookie crumbles

    Yes you obviously right. As you can see it was very late when I posted the question sorry for the silly mistake. I mean the total daily dose!! But if I spoke to the doctor and he agreed then I could go ahead with 50mg QDS instead? In the BNF they don't say about conversion between formulations so I assume it should be fine...?

    only if the dr is happy to change the script or get a new one to you...he may have gone for the BD dosing to make it easier for the patient..improve compliance and all that...

    i was told you should never assume....makes an ass out of u and me

    Yeah, the compliance, that's what I figured ....
    "assuming"? yeah, I know, I am still new at this I mean new at making decisions and being confident in applying knowledge... hopefully I won't be making an ass out of me during hospital pre-reg interview...if I get invited to one of course .... any advice perhaps... thanks

    be yourself, sell yourself, sound interested, don't just be pharmacy pharmacy pharmacy, show that there are other sides to yourself, hobbies, social activities etc. experience will help as well..but there are other threads in the forum on this topic

    Possibly we can still change the Thread Name to Nitrofurantoin from Trimethoprim. Surprising that in the first post it does say Nitrofurantoin but all other posts including the front page it says Trimethoprim ??????????
