Help needed clinical hospital patient question on drug chart

say on a drug chart you just looked at had the following drugs,

    aspirin
    cyclizine
    dalteparin
    diclofenac
    digoxin
    amiodarone
    ibuprofen
    Paracetamol

what would you say is wrong with the patient and what is wrong with the meds and how would you change them/monitor?

    I suspect an assignment !

Amiodarone and digoxin scream out AF
Aspirin with ibuprofen AND diclofenac - basic NSAID pharmacology will tell you the problem here.
By the way how would anyone know what's wrong with the meds when there's no dose, frequency, etc
Dalteparin is a LMWH, different indications, based on the list of 8 drugs you've given patient prob had (or is having) surgical procedure - cyclizine prob fits in well with this.

And please don't post same q on three different threads, one is enough.

Edit: Also on a drug chart you should see the dose, frequency and route of administration, e.g diclofenac suppositories, dispersable aspirin, etc. Need to be clear on these, particularly when some drugs have multiple indications with different doses.
Also please post up your ideas as well as asking others for theirs - you'll find it hard here unless we know you've done some work.

on 3 NSAIDs and also on LMWH... but I presume the LMWH is prophylaxis for DVT...should monitor renal function because of the NSAIDs, liver function and thyroid function because of amiodarone.. also monitor heart beats (if lower than 60 beats per minute, stop digoxin)...that's all I can think of really.,.
