He weights 10kg so 1000mcg 4 times a day which 4000mcg= 4mg. What is wrong with the script?

Salbutamol oral solution does not come in that strength --see BNF
It's unlicensed for use in children under 2 years old...
And if it did come in that strength, at 5ml QDS the child would be swallowing 10mg/day...

Sorry for the disturbance. I'm calling to check on the script you gave this morning Master Y. Salbutamol liquid comes as 2mg/5ml and you recommended 5ml qid that makes it 2mg a dose for a 10kg infant which is the double of the recommended 0.1mg/kg..I'm sure you have your reasons. I just wanted to confirm you're still happy for me to proceed with the script?
Dr: Make it 2.5ml qid...(what about saying sorry.. thanks...)
I don't think an apology is necessary (if the child had been harmed then maybe to the child/family, yes, but not to the pharmacist IMO, although thinking about it we'd have been almost as liable as the GP!)
A 'thank you' would be nice, if it wasn't forthcoming, I'd do what I do to my 3-year-old and say 'you're welcome' anyway![]()
