Question Re: mistakes in prescriptions!!

Had a script for:

1. Poorly written script had – metoclopramide written up for a pregnant lady. It was hard to read, but it also looked a bit like misoprostol. On questioning, it was supposed to be misoprostol…….


2. Diprobase cream to be mixed with hydrocortisone 1% cream in the pharmacy

The NPA told me there was insufficient stability data and the two should not be mixed…

A previous pharmacist had supplied this. Who mixes up creams in the pharmacy these days?
