It's happening - but it's very much a minority - GP still has to turn out multiple prescriptions - but does them all in one go.

You want to read this:

During my first three months of working, I encountered a horrible mistake. The prescriber wrote, BY MISTAKE, zestril instead of zyrtec for an adult patient. It is difficult to detect such error. A more experienced pharmacist asked me to check because that partilcular prescriber tends to write zestril instead of zyrtec!!!

I once had mysoline prescribed instead of triominic. Only picked up because was member of staff who had hay fever.

Often, but not always:
When I first moved to a new Pharmacy, I 'phoned one of the local surgeries' practice managers, to make an informal appointment to introduce myself; the reply was "Why me? I'm really not very medically-minded, and the 'Girls Downstairs' pretty much run the place on their own, anyway!"
So than whas the end of that working relationship.

And another local surgery sent us a handwritten Rx last week for "carbimazole 100mg, take 2 BD"... Should have been carbamazepine...

Seriously now, here are 2 errors that have occurred in my area:
-Cyclophosphamide 50mg ,one daily, dispensed instead of Cyanocobalamin 50mg, one daily;
-clomipramine 50mg caps instead of clomiphene 50mg tabs.
And I've seen a few scripts for normacol sachets, "use to clean wound"...

worst i had was when i had a script for neoral instead of neurontin. good job i had a pmr to look at. scares me to think that if they'd just walked in off the street as a new patient i would probably have dispensed it without much further thought...
