You want to read this: During my first three months of working, I encountered a horrible mistake. The prescriber wrote, BY MISTAKE, zestril instead of zyrtec for an adult patient. It is difficult to detect such error. A more experienced pharmacist asked me to check because that partilcular prescriber tends to write zestril instead of zyrtec!!! |

Seriously now, here are 2 errors that have occurred in my area: -Cyclophosphamide 50mg ,one daily, dispensed instead of Cyanocobalamin 50mg, one daily; -clomipramine 50mg caps instead of clomiphene 50mg tabs. And I've seen a few scripts for normacol sachets, "use to clean wound"...

worst i had was when i had a script for neoral instead of neurontin. good job i had a pmr to look at. scares me to think that if they'd just walked in off the street as a new patient i would probably have dispensed it without much further thought...
