Wonder why this wording can't be preprinted on the MDA form.
The prescriber can then have the option of striking it through and initialling it if it does not apply. My 'scriptcrime' was to dispense for an client with an excellent script and attendance record whose clinic had got mixed up with two bank holidays this Easter, and the script didn't have the variation stamp. Still waiting for the knock on the door...
