Could the pharmacist be in trouble for making a professional decision at odds with the law of the land - yes.
I've made similar decisions in the past - will continue to make them in the future.
I also inform the prescriber (or more usually their answer phone) of my actions.

I think you're not so scared of the inspector as me! Is it the law to have the set of words on the script - I thought it was? If it's not against the law, and a professional decision then what's the fuss about? If it is against the law, why are you doing it Jeff - aren't you putting yourself at risk?
