Re: mistakes in prescriptions!!
I was going through the owings this friday. There was an owing for glibenclamide 5mg 2BD. I checked the records, and this was the third month that the patient was having this dispensed. As this pharmacy is run by locums, it means that probably three different pharmacists had dispensed it.
I called the surgery to log the query it, I think that probably the doctor wanted gliclazide instead of glibeclamide (once a day dosage, max. 15mg a day). After waiting a few hours and then calling back, I was told that I should tell the patient that the GP wanted to book a medication review, but she could finish this course because if it hadn't harmed her yet, it would be unlikely to do happen anything in the next couple of weeks. Oh!, I didn't speak with the doctor, this was the message passed by the recepcionist.
