One of the 'white powder' incidents was when Sodium Fluoride was put in the Sodium Chloride stock jar. At that time Mist Diaphoretica (Sodium Chloride compound mixture) was popular. A pt tasted some but brought back as tasting 'funny'. Dispenser took a swig and was reputed to have died.
Result was that shop rounds were banned and we threw scores away in the rubbish.
